Instructions

Step 1

If you have a compulsory medical insurance policy, then it will not be difficult to move from one clinic to another. Just pick up your medical record at the reception of the institution from which you decide to refuse the services. Most likely, the new clinic will lead you to another one. But the old map will also come in handy for making and clarifying diagnoses.

Step 2

If you do not have a compulsory medical insurance policy, or you quit your job and handed over the document to the HR department, you will have to get the coveted card again.

Step 3

On May 1, 2011, new compulsory medical insurance policies began to be issued. They are a plastic card with a microchip. All the necessary information about the owner of the policy is encrypted on it.

Step 4

To get a new policy, you need to go to the issuing point, which is most likely in your clinic.

Step 5

If suddenly there is no point for issuing policies in a medical institution, contact the registry. There they will tell you the addresses of the nearest.

Step 6

To get the policy, you need to have with you:

- passport (to establish registration);

- certificate of registration (in the absence of a permanent residence permit);

- residence permit (for foreigners);

- a birth certificate and a certificate from the house administration about the registration of the child (when issuing a policy for the child).

Step 7

If all the documents are in order, the policy will be issued to you immediately.

Step 8

With a compulsory medical insurance policy, you can not only attach to another clinic, but also independently choose a doctor and an insurance company.

Step 9

To start serving you at another medical institution, contact the reception. Show your passport and medical certificate - you will be given a new card.

Step 10

If you are currently ill and you have already been prescribed treatment, be sure to inform the medical staff about it. This is necessary in order for you to be prescribed medications that are compatible with the previous therapy.

Step 11

Every time you want to make an appointment or contact the reception for other issues, you need to have your passport and compulsory medical insurance policy with you. Without these documents, the clinic staff have the right not to serve you.
