Instructions
Step 1
Find out which branch of the Pension Fund of the Russian Federation (PFR) serves your registration address. You can do this using the official website of the PFR (forms for searching for branches by list and map are in the upper right corner of each page).
Call your branch, specify its opening hours and the procedure for receiving citizens on the issue of self-registration of a certificate of compulsory pension insurance.
Step 2
The insured person's questionnaire form (ADV-1) in pdf format for Moscow and doc - for other regions you can download on the PFR website and fill out at home on a computer or by hand. Or take it from your branch of the FIU.
You need to fill out the questionnaire in blue or black ink.
If you live in the same place where you are registered, you do not need to fill in the field about the actual address.
Step 3
If you filled out the questionnaire at home, come to your PFR office with your passport. When issuing a certificate not at the address of registration, grab, if available, a document confirming registration at the place of stay.
After accepting the application and checking it with the data of the passport and other documents, the employees will inform you of the time when you need to come for a ready-made certificate.
