Step 1
It is best to write a statement directly at the police station, this will save time and effort in formulating the text itself. The main thing is to listen carefully to the person on duty, delve into the meaning of the statement and write only what you completely agree with. If you decide to write an application yourself at home, then it is better to print it on a computer using the standard scheme.
Step 2
In the upper right corner, you need to indicate the police department to which you submit the application, for example, "To the duty station No. …", then enter the official, you can write in a general way, but it is much more effective to indicate the name of the chief.
Next, you need to enter all your data: name, address and contact information, write in the center in large letters the word "Application" and go directly to the presentation of the existing problem.
Step 3
Despite the fact that further text is not regulated, it must be written clearly, unambiguously and structured. There should be no ambiguous phrases and unnecessary information, the essence should be stated briefly and clearly, paying attention only to the facts.
Step 4
It is necessary to indicate the estimated time of the crime, identify possible witnesses and make an assumption about the identity of the offender, if you have objective grounds.

At the end of the application, it makes sense to make a reference to the text of the Criminal Procedure Code of the Russian Federation, paragraph 1 of Art. 145, which describes the duties of the investigator, interrogator and the body of inquiry, which will show your legal knowledge, and therefore, deprive the person on duty of the idea of rejecting your application.
Step 6
Below it is necessary to fill in the information that you are aware of the responsibility for knowingly false denunciation in accordance with Art. 306 of the Criminal Code of the Russian Federation, since this can also become a reason for refusing to accept an application.
Step 7
The end of the text must be dated and signed. Moreover, the signature should be located directly at the end of the text so that it would not be possible to enter anything there.
