Since there is no obligatory form for it, it is necessary to be guided, first of all, by common sense.

Instructions
Step 1
At the top of the sheet on the left side, write the name of the territorial body of internal affairs for your district to which you will apply. Also include your last name, first name, patronymic, passport details, actual address, contact information: home and mobile phone numbers, fax, e-mail address.
Step 2
Below, in the middle of the sheet, write "Statement of theft of money."
Step 3
In the main text, describe the essence of the incident in any form. Indicate in what period of time and from where the money was stolen. Check if we are talking about the theft of cash or non-cash funds from a bank card. If you suspect someone of this crime, please provide information about that person or persons.
Step 4
At the end of the statement, state your request to open a criminal case on the theft of money and find the culprit. It is not required to refer to the norms of the law. Also, ask for the number under which your application will be registered.
Step 5
Sign the application, put the current date. If a document is submitted from an organization, it is signed by its head, the signature is certified by a seal.
Step 6
Submit an application to the territorial body of internal affairs in your area in person or through an authorized representative, to whom you should issue a power of attorney. You can also send the application by mail, fax.

If it is more convenient for you to contact the police via the Internet, use the website www.112.ru, in the "Urgent communication" section, select the link "Report to law enforcement agencies". Then, in the field that appears, select the department "Ministry of Internal Affairs of Russia". After reading the rules, put a tick at the bottom of the page opposite the words "I have read and agree with the rules and procedure for contacting the Ministry of Internal Affairs of Russia." Fill out the e-form that appears. Please note that fields marked with an asterisk are required.
