Writing a threat statement begins with filling in the so-called "header", which is usually located in the upper right corner. It indicates the name of the body or official to whom you are applying, his rank or class rank, surname, name, patronymic of the applicant, address of residence, and, if possible, indicate the contact phone number.

Further, in the center of the stock, the word "Statement" is written. Then, in a free style, you need to state the essence of the danger that threatens you. It is better to compose the text from small clear sentences that contain only facts that matter. It is better to avoid all kinds of expressions like "maybe", "probably" in the statement. If you have documentary evidence of an imminent danger or an already committed crime, attach copies of these documents, indicating their list and the number of sheets in the text of the application.

After stating the essence, indicate your request to consider your application in accordance with the norms of the current legislation (in the text you can make references to the relevant articles) and conduct a procedural check on the facts specified in the application. You can add a sentence to give written notice of a decision made on your application. It is imperative that you do this so that you have the opportunity to appeal if you disagree with the results. It's just that in practice this rule is very often violated, and the applicant sometimes remains in the dark for a long time, and then precious time is lost, and it becomes difficult and sometimes impossible to achieve his goal.

The last line in the application must indicate the fact that you have been warned of criminal liability under Art. 306 of the Criminal Code for knowingly false denunciation. With this entry, you will deprive the police officer of the reason to reject your application due to the possible false denunciation and lack of any responsibility for this. Then sign the application, decipher your signature, indicating next to your last name and initials, put the current date.
Step 5
Take the completed application to any nearby police station and contact the officer on duty to be accepted. Do not give in to excuses such as contacting the scene of the crime or that statements are being made at a specific time. According to the instructions, messages are received and registered around the clock and in any department. If the crime is committed in another place, your application will be sent to the area where it should be considered on the merits.
