Nowadays, a lot of people are in business. In order to formalize your business, you need to fill out a number of documents. One of them is an application for state registration of a legal entity for creation. It consists of three sheets and appendices A-H.

Instructions
Step 1
In the form, fill out the first sheet, where indicate the name of the registering authority and its code (depending on the location of the tax office). Indicate the organizational and legal form of your organization (limited liability company, individual entrepreneur, etc.)
Step 2
Write the name of the enterprise (full and abbreviated in Russian, the language of the peoples of the Russian Federation and a foreign language, the item in Russian is mandatory).
Step 3
Tick the place of state registration of the legal entity, namely: the address of the permanent executive body, other body or person entitled to act on behalf of the legal entity without a power of attorney. Indicate the name of the authority, write down the address in detail.
Step 4
Write the phone number of your organization with the area code.
Step 5
Indicate the number of founders of the legal entity.
Step 6
Check the box who is the founder (legal entity, individual, Russian Federation, constituent entity of the Russian Federation, municipality, owners of investment units of a mutual investment fund) and fill in the information about the founder in the corresponding application of the application.
Step 7
Indicate information about the authorized capital (share capital, authorized capital, mutual fund) and tick the appropriate box.
Step 8
Write the size of the authorized capital in rubles.
Step 9
Enter information about the register of the holder of shareholders of the joint-stock company in Appendix D of this application form, if your organization belongs to JSC or CJSC.
Step 10
Enter information about the managing organization in Appendix G of the application, if the powers of the executive body of your organization have been transferred under an agreement to another commercial organization.
Step 11
Indicate the number of persons entitled to act on behalf of a legal entity without a power of attorney. Write down their details in Appendix E of this statement.
Step 12
Information about the manager - an individual entrepreneur - enter in sheet Z of the application.
Step 13
Indicate the number of separate subdivisions of the legal entity: branches (write down information in Appendix I), representative offices (write down information in Appendix K), if any.
Step 14
Indicate the number of peasant (farmer) households, on the basis of the property of which a production cooperative or economic partnership is being created (write down the information in Appendix L of the application).
Step 15
Indicate the number of types of economic activities (enter information in Appendix M of the application).
Step 16
In Appendix H of the application, enter information about the applicant, fill out two pages, the third - in the presence of a notary.
