Instructions
Step 1
When you contact the FMS of Russia at your place of residence, you will be given an application form for issuing (replacing) a passport. It must be filled in legible handwriting. In the first line, you must write the last name, first name and patronymic, for example: Elena Ivanovna Petrova.
Step 2
Write your date of birth: day, month and year. For example: April 30, 1984.
Step 3
Indicate your place of birth. It can be a village, settlement, city, district, region, region. For example: Moscow.
Step 4
You must indicate your gender. For example: Female.
Step 5
Indicate your marital status. If you are not married: not married / not married. If you are married, you must indicate the last name, first name and patronymic of the wife (husband), which registry office registered the marriage. For example: Petrov Oleg Andreevich, the Nagatinsky department of the registry office in the city of MOSCOW.
Step 6
Surname, name, patronymic of father and mother. Indicate the initials of your parents: Krasnov Ivan Anatolyevich, Krasnova Tatyana Sergeevna.
Step 7
Place of residence, stay. Cross out the unnecessary. In this line, specify the name of the region, district, your settlement, street name, house number, building and apartment. For example: Moscow, st. Nagatinskaya embankment, 62, apt. 24.
Step 8
In the eighth line, you must indicate whether you were previously in a foreign state and when you acquired Russian citizenship. This applies to those who were previously a citizen of another country. If this does not apply to you, write: was not (la). If you have previously lived in another country, indicate its name and the date of your adoption of Russian citizenship. For example: United States of America, February 18, 2001.
Step 9
In the next line, indicate why you need to issue (replace) a passport. The reason may be a change of surname, reaching a certain age, loss or theft of a document. For example: I ask to issue (replace) a passport in connection with a change of surname. Please indicate the date of filling out the application and your signature below.
Step 10
On the next line, write the details of your old passport: series, number, when and by whom it was issued.
