The procedure for filling out a questionnaire for issuing a passport is not particularly difficult. You just have to enter your personal data or make notes in the right places. The application can be completed on a computer, on a typewriter, or in legible handwriting (preferably in block letters).
It is necessary
- - birth certificate or old passport;
- - document on registration at the place of residence or stay (if any);
- - a fountain pen, typewriter or computer with a printer.
Instructions
Step 1
Do not fill in the topmost field (FMS department code, passport series and number, date of issue), this will be done by FMS employees. You should start from point 1 - "Surname, name, patronymic".
Fill in all columns in strict accordance with how this or that information is recorded in the birth certificate or old passport.
In column 3 (gender), check the box next to the desired value. In column 6 (marital status) write “single” or “not married” if you are not married. Otherwise - "married" or "married" and the name of the wife or husband.
Step 2
In the column on the place of residence, indicate the address of registration (registration at the place of residence). If not specified anywhere, leave this field blank.
If you have a registration and you submit documents to its address, column 7.1. leave blank. If not at the place of registration, but for temporary registration (registration at the place of stay), put a tick in front of the item "At the place of stay" and indicate the address at which you are temporarily registered. When submitting an application outside the place of registration and in an area where you do not have temporary registration, tick the option "at the place of application" and indicate the address where you live in this area.
Step 3
If you have not been a foreign citizen, leave paragraph 8 empty. If you were, indicate which country you were a citizen of, and the date of adoption of Russian citizenship.
In paragraph 9, indicate the reason why you are asking for a passport. Depending on the situation: for the first time, change of surname, gender, change in appearance, loss or damage of a passport, etc.
Do not rush to sign the application. Do this in the presence of a passport officer who must certify your signature.
Do not fill in anything below paragraph 10: this is for service marks.
