A trip to a children's camp is a good chance for your child to show their independence, make new friends and significantly expand their horizons. If you decide to send your child to a summer health camp, you should carefully read the instructions on how to get a voucher.
Instructions
Step 1
Since 2010, the authority to organize summer vacations for schoolchildren has been transferred to regional authorities. Therefore, to obtain a voucher, contact the departments of education or social protection of the population if your family has a certain social status (registered with social protection institutions). There is no single algorithm of actions for all regions, so find out the details of obtaining a voucher when contacting an educational institution. either to the regional education authority, or, as mentioned above, to the committee for social protection of the population.
Step 2
When applying for a voucher, do not forget to find out what are the conditions for its provision. Each region sets the level of the surcharge. It can vary from a purely symbolic 5% for a voucher (for children from low-income families or public sector employees) to 90% (for families with a high income).
Step 3
You can also get a holiday camp through the Social Security Fund. Contact the social security commission set up at your business or contact your business manager directly. Write an application for a voucher. Since the state has obliged the policyholders to improve the health of children of working parents, you are required to provide a voucher or compensate for a set percentage of the cost of the voucher purchased independently upon presentation.
Step 4
To receive a voucher, attach a copy of the child's birth certificate to the application; registration document; a copy of the passport of one of the parents and, if necessary, a certificate of the total family income. If your family has benefits, prepare a document confirming them.
Step 5
To issue a dedicated voucher, prepare the following package of documents: a medical certificate with notes on the preventive vaccinations carried out; conclusion on the epidemiological environment (no quarantine); a copy of the insurance policy. Remember that the voucher is valid only if you have a completed medical card with a note about all past illnesses and chronic forms of your child's illnesses.
