Step 1

Tell your employer if you lost your pension insurance certificate. Your employer is obliged to apply to the territorial body of the Pension Fund with an application form ADV-3. Step 2

Please note that the application form ADV-3 indicates SNILS - your insurance number of an individual personal account, which was assigned to you when issuing a lost certificate. If you do not remember SNILS, and the employer did not retain this information for some reason, SNILS will have to be requested from the Pension Fund, or at your previous place of work.

Step 3

Wait about a month until a duplicate of your certificate is ready. Get a duplicate. Check the correctness of filling in all the details. Sign your employer's record of receipt of your certification.

Step 4

Obtain a duplicate of the pension insurance certificate yourself at the territorial body of the Pension Fund of the Russian Federation, if your employer refuses to deal with this issue, or if you do not have a permanent job. And also when it comes to your child's pension insurance certificate.

Step 5

Come with your passport to your territorial office of the Pension Fund. If the insurance certificate of a child under 14 is lost, take his birth certificate. Fill out the application form ADV-3 for the issue of a duplicate. The document shall be completed legibly in block letters by hand, in ink of any color other than red and green.
