Step 1
After reaching the age of 16, within 1 month, apply for a passport to the department of the State Migration Service of Ukraine at your place of residence. Step 2
In case of loss or theft of a passport, additionally submit the following documents: a statement about the loss of a passport, a certificate from the housing office about registration. Pay the state fee for issuing a passport in the amount of 34 hryvnia. If your passport is stolen, take a certified copy of the extract from the relevant criminal proceedings to the police. Please note that the application should state the circumstances under which the passport was lost or stolen.
Step 3
The passport must also be replaced if it is damaged or becomes unusable, if an inaccuracy is found in the entries in it, as well as due to a change in the person's surname, first name, patronymic. In this case, provide the passport to be replaced, as well as documents confirming the change of initials.
Step 4
When restoring a passport, copies of documents are required, on the basis of which appropriate marks are put down in it. These documents include: marriage or divorce certificates, as well as birth certificates for children under 16 years of age. A mark on the presence of a foreign passport is made by a subdivision of the State Migration Service independently.
Step 5
The standard period established by law for the production of a passport is 1 month. For this period, issue a temporary identity card for yourself, which, after receiving your passport, will need to be handed over.
