Step 1

Fill out an application form for a passport. Do it at the UMS for Udmurtia (Izhevsk, Pushkinskaya str., 164) for an additional fee. Or download the form on the website of the Office of the Migration Service and fill it out yourself. Please note: in order to download the new passport application form, you will need the Adobe Reader program.

Step 2

Fill out the application on your computer. All information should be as complete as possible. Do not make corrections and mistakes. All form fields are filled with capital (capital) letters. Print out the completed application form in two copies (for registration of a child's passport - in one).

Step 3

Enter in the column "Information on labor activity for the last 10 years" records of education (including school education), extracts from the work record book, military ID, etc. In this case, you will no longer need a copy of the work book. Ask the head of the organization where you work or study to sign this box. If you are currently a member of the labor exchange, you will have to bring a work book to the UMS for verification, a certificate from the employment service and indicate in this box the address of your place of residence.

Step 4

Do not forget that you will need a passport of a citizen of the Russian Federation (birth certificate of a child with a mark of citizenship) and its certified copy (pages with registration and photograph). Young people from 18 to 27 years old will also need a certificate from the military registration and enlistment office. Take 2 photographs for your passport in the UMS, each of which will be attached to a copy of the questionnaire (children, respectively, will need only 1 photograph).

Step 5

Find out at the UMC department the details and pay the state duty in the established amount. Give the migration service inspector the completed application form along with the documents. Within 30 days, you will be given a passport if all the information you specified in the application is true.
