It is necessary

- - a notarized power of attorney and a copy of the client's passport when receiving a document for another person.
Instructions
Step 1
Fill out an application for a renewed birth certificate. Its form can be downloaded from the websites of regional offices of the registry office, regional portals of public services, if available in the region, and the federal portal "Gosuslugi.ru".
Step 2
Along with the application, also present documents confirming your right to receive this document. If you want to get your own certificate, a passport is enough. For the document of a deceased relative, you need his death certificate and confirmation of your relationship: birth certificates, marriage certificates, etc. When applying on the basis of a power of attorney - a copy of the client's passport and the original of a notarized power of attorney.
When applying by mail, attach copies of all necessary documents to the application and send it in a valuable letter with a list of attachments and return receipt.
Step 3
You can check the amount of the state duty and the details for its payment with the registry office of the region of interest.
Step 4
If you personally contact the registry office, the document will be issued to you on the same day.
They are obliged to take measures for written application within 30 days from the date of receipt. In this case, the certificate will be sent to the registry office at your place of residence, about which you will be notified by mail.
