Decide why you need a copy of your medical history. In some cases, for example, to undergo a medical examination to establish a disability, an extract from the documents with the diagnosis and course of the disease will be enough for you. If you suspect doctors of unfair treatment, then you will need copies of all documents, including tests and X-rays.
Step 2
Copies of the necessary documents should be given to you free of charge and at your request. Contact the receptionist and ask him to make a copy. However, you must have your passport with you to verify your identity. If you are denied for any reason, make a written request to the name of the chief physician or director. In the header of the request, indicate the last name, first name, patronymic and position of the addressee, the coordinates of the medical institution. In the main text, indicate your request - the issuance of a copy of the medical history, put your name and signature. You can also refer to the federal law "On Personal Data", which obliges medical institutions to provide citizens with information about the course of their treatment.
