Step 1
Choose the child's first name, as well as the last name (provided that the parents have different last names) and patronymic (if the child is drawn up by a single mother). If the procedure for establishing paternity is not initiated, then the child is registered in the mother's surname. In this case, information about the father is filled in according to the mother's words, or is not filled at all. Single mothers who want to establish paternity will have to pay a state fee of 100 rubles.
Step 2
Come personally to the registry office at the place of your registration or at the place of birth of the child. Or to the diplomatic mission of the Russian Federation - if you are abroad. With you, you must have a certificate from the maternity hospital (or other medical institution) of the established form - it is given to a young mother upon discharge. If the parents are legally married, then the personal presence of only one of them is sufficient for registration. At the same time, you will need to have a marriage certificate and passport of the second spouse. If the spouses come together, a marriage certificate is not required. If the marriage is not officially registered, then both parents must be present at the registration of the child. If none of the parents (for some reason) can personally be present, then a power of attorney is drawn up for one of the relatives or employees of the medical institution to register the baby.
Step 3
Fill out the application form in the established form, which will be given to you right in your office. Wait while the registry office employee checks the correctness of your application and reviews the documents you brought. If everything is correct, then you will receive a birth certificate of the child in your arms within about 10 minutes.
Step 4
Keep both your own birth certificate and those of your children. If you have lost the document, then contact the registry office with an application for re-issuance of a birth certificate. You need to have a passport and documents on the change of surname (if it happened). For the issuance of a repeated document, you will have to pay a state fee (100 rubles). It will be possible to get a duplicate birth certificate in about a month.
