Instructions

Step 1

Accept from the employee an application for amending personal data in connection with a change in the last name (for family and other reasons). Issue an order on the need to make changes to all employee documents that are managed by the HR department and accounting. The order must be registered in the Order Log. Familiarize the personnel department, accounting department and the employee himself with the order.

Step 2

Before making changes to the employment contract, a voluntary agreement must be entered into with the employee. After the agreement is concluded, amendments can be made to the contract by crossing out the old name and inscribing a new one above the corresponding column. To confirm the legality of the amendments, indicate the name and number of the document on the basis of which the amendments were made (for example, certificates of marriage / divorce). After that, the data changed in the "Surname" column are certified by the employer and the employee.

Step 3

Based on the documents confirming the legality of changes in the employee's personal data, amendments are also made to the work book (on the first page or title page). The previous surname is crossed out with one line, after which a new one is indicated at the top of this entry. Links to the relevant certificates (and other documents) are indicated on the inside of the cover of the work book. And it is this record that is certified by the signature of the employer and the seal of the HR department.

Step 4

In the employee's personal card, his previous surname is crossed out with one line and a new one is indicated next (or above). The name and number of the document, on the basis of which the changes are made, are indicated opposite the column "Surname". Information about the document can be entered in the X column of the personal card ("Additional information"). These amendments must be certified by a human resources officer.
