Step 1

Fill out the notification form using a ballpoint pen, not a pencil. When writing, press the pen firmly into the paper so that the copy of the notice is clearly legible. Please note that the front part of the notice must be filled out together with the second participant in the traffic accident. In this case, both drivers must sign both copies of the notice. If the participants in the accident have strong disagreements in the assessment of what happened, an option is allowed in which each driver will fill out his own set of forms (original and copy).

Describe in detail the number, color, as well as the make of the car involved in the accident, if its driver refuses to draw up a notice with you. Please indicate any disagreements with the second participant in the "Notes" item. Also in the form, write down the details of the accident: the diagram of the scene of the accident, the location of the cars at the time of the accident, the road marking line, the location of the nearest road signs, the trajectory of the skid and the area where debris or vehicle parts are scattered.

Step 3

Record all accident witness data, if any. If not, check the appropriate box. If possible, certify the notice from the traffic police officer who arrives at the scene of the accident, ask him to write his last name, first name, patronymic, position and contact information. Remember that without a protocol drawn up by a traffic police officer, the notice will not have legal force.

Step 4

List in detail any damage your vehicle has sustained. Remember that they must also be recorded by a traffic police officer. In point No. 13, indicate the location of the impact of your car. Track exactly what damage is described by the second driver, and whether they coincide with real ones. If the second participant in the accident does not agree with your description, please mark this in the "Notes" column.

Step 5

Indicate the series and number of your insurance policy and the name of the insurance company, as well as the details of the second participant in the accident. Finally, disconnect the notice forms, fill in the reverse side. If there is not enough space on the form, add the rest of the information on a blank sheet of paper, making a note on the form about the attachment.
