If you were awarded a medal, an insignia, or a breastplate testifying to your services to the state, then losing it will be very unpleasant. However, no one is immune from such a situation that can occur when moving, natural disaster, or theft. Nevertheless, if it did happen, do not despair: the lost medal can be restored.
Instructions
Step 1
So, if you have lost an order, medal, insignia or badge to an honorary title, you need to write a statement addressed to the head of the administration of the constituent entity of the Russian Federation in which you live: republic, territory, region, autonomous region or okrug. If you live in Moscow or St. Petersburg, apply to the mayor of the city. In your application, please describe in detail the circumstances under which the state award was lost. You must attach the awarding documents to the application.
Step 2
If the award documents were lost along with the award itself, send a request to the Central Archives of the Ministry of Defense, which is located in Podolsk. It contains documents of all military units of the state, starting from 1941. You will be given a duplicate of the award document or a certificate of award.
Step 3
If, after considering the circumstances of the loss of the state award, the head of the regional administration agrees that you were not able to prevent it, he will send a petition to the Commission on State Awards under the President of the Russian Federation. By her decision, you will be given a duplicate of the award or a dummy.
Step 4
If only the award document was lost, apply to the head of the local government of the city or district in which you live. They will be sent a petition to the Office of the President for Personnel Issues and State Awards. Along with this document, your application and a certificate on the reason for the loss of the state award are sent to the Office of the President.
Step 5
If you have lost the award documents as a result of hostilities, natural disasters or other reasons beyond your control, you will be given a duplicate of the award document. In other cases, get a certificate of awarding a state award.
