It is necessary

Instructions
Step 1
Apply to the reception of the polyclinic with your passport and medical certificate. Ask for a duplicate of the pages you want or your entire medical record. In most institutions, this service is provided free of charge and without any problems.
Step 2
Write a statement addressed to the head physician of the medical institution. Describe in detail the reasons why you need to have a medical record in your hands. According to the letter of the Ministry of Health and Social Development of the Russian Federation "On the procedure for keeping an outpatient card" dated April 4, 2005, this document can be handed over to you only if the head physician of the medical institution gives permission for this.
Step 3
If you pick up a medical card because you move to another city and detach from this clinic, then also write a statement to the name of the chief physician, the form of which can be obtained from the registry or the office of the hospital. Indicate the reason for your “detachment” to change your place of residence.
Step 4
If you want to take the medical record of a deceased relative, it is unlikely that they will give it to you, referring again to the above letter dated 4.04.2005, which says that “The medical records of deceased people are withdrawn from the current card file for transfer to the archive of a medical institution, where 25 years . But if you have good reasons for demanding the issuance of a card in your hands, resolve this issue through the head physician.
Step 5
If you go to the hospital for treatment or examination, the medical record, according to the law, is transferred by the employees of the medical institution to the above-mentioned medical institution and then, after discharge, is returned back without the participation of the patient.
Step 6
If you need a medical card for the examination prescribed by the doctor of the polyclinic in another hospital, then it will be given to you without any problems, you just have to show this appointment, certified by the doctor.
