Step 1

Take a piece of A4 paper. Some organizations have their own complaint forms - you can use them. The complaint must be made in two copies. One is given to the institution against which you are writing a complaint, and the second remains with you.

Step 2

Indicate in the upper right corner the name of the organization to which the application is being sent. The position, surname, name and patronymic of the head of the organization are noted below in the dative case. Even lower, in the genitive case, your surname, name and patronymic, address and telephone number are indicated. This is necessary for feedback from you, as anonymous complaints are usually not considered. On the left side of the sheet, you must indicate a summary of the complaint. For example, if you are complaining about the work of an employee of the ZhEK, then in this line write: "About illegal actions of an employee of the ZhEK." Write the word "Complaint" in the center of the sheet.

Step 3

Write your complaint text. In the description, you must indicate the maximum information you know. Indicate the full name, position, rank, place of work, badge number, official ID number, address and other data of the person to whom the complaint is being written. Describe the circumstances in which your rights were violated, indicate the persons who can confirm the violation. If possible, provide the exact name of the violation of consumer rights, rules or laws. If you have documentary evidence, please attach copies certified by a notary to the complaint. At the end of the complaint, be sure to indicate a list of the attached documents, below - on the left, put the date of writing, and on the right - a signature with a transcript.

Step 4

Send the complaint to the receptionist of the organization. Ask to put an incoming stamp on the complaint and your copy, make sure that the dates on both copies are the same. A complaint is usually reviewed within a few days, and a response can be expected within a month.
