Step 1

If you are employed, then you need to apply for the exchange of insurance certificate to your employer. This must be done within 30 days from the date of the change of surname, name or patronymic. You will need to attach to the application:

- a copy of your passport;

- a copy and original of a document that confirms the fact of a change of surname, name or patronymic (marriage certificate, court decision).

The employer, within 14 days from the date of receipt from you of the application for changing the insurance certificate, will transfer it to the branch of the pension fund with a certified covering letter of the organization. Within 30 days, you will be issued a certificate of compulsory pension insurance with new data.

Step 2

If you do not work, you must independently apply to the pension fund authority at your place of residence, fill out the appropriate form for the exchange of the insurance certificate of compulsory pension insurance and attach the following documents:

-copy of the passport;

- a copy and original of a document that confirms the fact of a change of surname, name or patronymic (marriage certificate, court decision).

Within 30 days, you will be issued a new certificate of compulsory pension insurance.
