The passport is changed upon reaching a certain age - twenty and forty-five years. It is necessary
- - old passport of a citizen of the Russian Federation;
- - an insert about citizenship;
- - two photographs 35x45 mm in size, color or black and white;
- - marriage certificate;
- - a document certifying the change of name or surname;
- - receipt of payment of the duty;
- - application for changing the surname to another (in case of marriage);
- - a questionnaire (issued by the Federal Migration Service of the Russian Federation).
Instructions
Step 1
Contact the authorities of the Federal Migration Service of the Russian Federation within a month from the date of expiry of the passport, that is, after reaching the age of replacement, marriage, change of surname or damage to the document. Otherwise, penalties will follow, and a new passport can be obtained only after their elimination. Step 2
Give all collected documents to the employee of the Federal Migration Service of the Russian Federation. He will give you a questionnaire, which you fill out on the spot and return back. Write in the appropriate paragraphs the last name, first name, patronymic, date and place of birth, gender. Indicate your marital status, last name, first name and patronymic of your parents, registration address and region of obtaining a civil passport. Note the reason why the document is being replaced. Sign and decipher the signature. On the reverse side of the form, insert the previous personal data (last name, first name, patronymic, number and date of issue of the previous passport). In the required column, write down the series, number and date when the foreign passport was issued, if any.
Step 3
Come to the office of the Federal Migration Service of the Russian Federation in two months if you are not registered in Moscow, your registration is temporary or there is none at all. During this time, department employees will send a request to the management to which you are assigned. After receiving a positive answer and an extract from the register, you will be issued a new civil passport.
